Up to 5 sessions per calendar year (maximum)

For patients with a chronic medical condition with complex care needs.

A chronic medical condition is one that has been (or is likely to be) present for six months or longer. It includes, but is not limited to, conditions such as asthma, cancer, cardiovascular disease, diabetes, musculoskeletal conditions, and stroke.

Patients have complex care needs if they need ongoing care from a multidisciplinary team consisting of their GP and at least two other health or care providers.

Can be provided to residents of an aged care facility provided both the GP and aged care facility have jointly contributed to a multidisciplinary care plan.

Requirements:

· GP Management Plan (Item 721 or review item 732) and

· Team Care Arrangements (Item 723 or review item 732)

· N.B. the referring GP must indicate the number of sessions allocated to each identified allied health service

Eligible allied health professionals:

· Dietitian

· Exercise Physiologist

· Mental Health Worker

· Occupational Therapist

· Osteopath

· Physiotherapist

· Podiatrist

· Speech Pathologist

A written report is provided to the referring GP after the first and last service, or more often if clinically necessary.
